DeMolay Forms


Following is a list of forms frequently used in chapter administration.

2008 Advisory Council Registration Process
PDF Adult Worker Application Form (48K)
This is the basic information form that must be submitted for all new Adult Volunteers.
PDF ACR - Continuing Advisor Form (41K)
If you were a registered advisor in 2007, your chapter should have received an ACR Continuing Advisor Form pre-printed with your name. Use this form if the pre-printed form has been lost or destroyed.

Scholarships for DeMolay Members

PDF DeMolay Scholarship Application Form (92K)
Use this form to apply for the DeMolay Foundation's Frank S. Land Scholarship, The Grotto Scholarship, or the York Rite Grand Chapter Royal Arch Masons Scholarship. Get more information. Applications are due by April 1.

DeMolay International Competitions

PDF Chapter-Challenge Bowling Tournament Information and Entry Form (201K)
(formerly called "Postal Bowling Tournament") The Chapter-Challenge Bowling Tournament is an annual contest held from November through January. Chapters take part locally and submit their results to be ranked against other chapters from around the globe.

PDF 2008 Kach DeMolay Essay Competition (144K)
Each year, the Kach Essay Competition awards $300 to each Region winner (two winners from each of DeMolay's eight regions) and $1,200 to each of the two national winners. For the two national winners, that's $1,500 each! More info here.

For I.S.C. Members

PDF ISC Membership Nomination Form (98K)
Form used to nominate someone to membership in the International Supreme Council.

PDF ISC Member Certificate of Change / No Change (81K)
Form must be filled out and signed by all Active (Class 1) and Deputy (Class 3) members annually in order to maintain their ISC membership. This form is sent to all ISC members with the annual membership invoice. It is provided here in case the original has been misplaced.
